Run this and get a look of broken glass over your image.
SUPPORT INFORMATION
Open your image and run Filter > Distort > Displace after making layer zero & a dupe layer. If you like you can use a layer mask to control where it appears.

I have tried this but to no avail.
What should the settings for horizontal and vertical be ?
stetch to fit or tile ?
Edges wrap or repeat ?
I can't seem to get the effect no matter what I try.

Works fine. This is just what I needed!! To get it to work, I opened a photo then Filter>Distort>Displace and chose the displacement map.
