TSPhpFeedback is a component developed for Dreamweaver MX and later versions. TSPhpFeedback allows the insertion of a feedback mechanism capable of sending back to the webmaster or propietary of the website. TSPhpFeedback is based on Php script to send the feedback e-mails and also uses AJAX technologies, widely supported by the main web browsers ( Internet Explorer 5.0+, Safari 1.2, Mozilla 1.0 / Firefox, Opera 9, and Netscape 7). Because of this, you don’t have to mind about your website been reloaded when sending the feedback e-mail. The Php script use requires that the Web Server that hosts your website allows Php running and configurated to allow the sending of e-mails from the proper script of the website. (In Dreamweaver, the Document Type Definition should be specified for the extension to work properly)

UI Access:
TSPhpFeedback is distributed as a Dreamweaver MX component in a .mxp file named TSPhpFeedback.mxp. By running TSPhpFeedback.mxp. the component will be installed and you’ll be able to access it (after restarting Dreamweaver) in the Commands menu
"Commands => Insert TSPhpFeedback".
(In Dreamweaver, the Document Type Definition should be specified for the extension to work properly)

It’s an unobtrusive feedback form. It isn’t free but is a fair price for a multi-domain license.
