HotDreamweaver PHP Toolbar is a collection of HotDreamweaver's eight most popular extensions. This Toolbar is designed for work with forms on servers supporting PHP scripts. The toolbar includes the following extensions: Form2Mail, FormValidator, FormCaptcha, Form2Excel, CalculatedFields, RelationalList, CheckAllForm, and UploadInBackground.

UI Access:
Simply double click the MXP file. Then restart Dreamweaver and once it is loaded, the new Toolbar will already be available.

it's good to have a complete set of form tools into a single product, I'm currently satisfied with the tools and the support

I have found all the extensions present in this toolbar separately, but of course the toolbar bundle is less expensive.
