The book club survey
Suppose you are the moderator of a large book club in your community. You want to conduct a Web-based survey and record the results in a database. The group has just finished reading Great Expectations, and you want everyone in the group to respond to the question "Who were your three favorite characters in the novel?"
You want to give your survey participants six response choices. Ideally, your survey form would look something like this:
