Planning your survey page
Creating this type of page takes some planning. You must begin by sketching out both your survey page and the database that will hold the survey information.
In this example, the database will consist of a table with four columns that correspond to the following four elements:

The database table will look something like this:
Values for each of the four elements will be added to the database when the user clicks the Submit button on the survey form.
