How to create a fillable PDF form with Acrobat:
- In Acrobat XI, select the Tools pane > Forms > Create.
- Select Existing Document, and click Next.
- Select a file to convert, and click Next.
- Select Tasks > Edit Fields to make adjustments to the form fields if needed.
- Click Close Form Editing when done.
